what prompted me to write this article is when i saw a big ad in Indian news paper about ROBOTIC HAIR TRANSPLANT. I tried to find out then came to know that this is the same company which was advertising about some other technique last year. They closed the shop and came up with new name this year. Indians  gets attracted by fancy names and fancy ads. where the world knows only two technique FUT and FUE Indian companies and clinic comes every month with new technique and names. With every new ad the hope raises in patients that something new has come up in the world for hair transplant. Patient are fleeced left right and center.

Lets know the truth about robotic hair transplant.

ARTAS system is one of the tool which has assisted surgeons in follicular extraction. I will call it a half robot as it can only help surgeon in extraction but rest of the procedure is dependent on surgeon skills.

There is a machine called NEOGRAFT which assists in graft extraction but its not a  ROBOT. This machine was launched in India long time back as MEDIKAMET and then withdrawn. Later it came with the name of NEOGRAFT

So what are these so called Indian robots. They are motorized tools to be held in hand and which assists surgeon in graft extraction. None of these procedures can do 4000 grafts extraction in one go which experienced hands can do.

Patients should be aware of these kind of tricks. They can safeguard themselves only by doing due research before going to any clinic. Let’s calculate how many grafts are required for a good density for any hair transplant. We have a normal density of 70-100 grafts per sq.cm.Looking at this we may have 50-75000 grafts (not hairs) in the entire scalp. Let’s assume that you have lost ZONE 1(first 2cm of front hair line) and ZONE 2 (next 5 cm of hair line).

To calculate the number of grafts needed you have to know the area which is empty. To calculate the hair line density for first two cms measure the area from left to right of the hair line. Usually it’s between 14-18 cms. If we take a average of 16 cms the area becomes (16×2) ie 32 sq cms. Now look at the density which you need. I recommend 50-70 grafts per sq cm for hair line including temple closure as that’s what will decide your hair transplant results. So a good dense hairline will need somewhere between 1900-2200(density neededx32) grafts.

Now look at the ZONE 2 which is another 5 cms behind it. We usually don’t need a density of 60-70 grafts per sq cm in zone 2.But even a density of35- 40 grafts per sq cms will consume 1500 -2000 grafts. So even you have hair loss in front two zones you will need somewhere between 3500-4000 grafts for good density.

Creating good density is also and an art and will vary from surgeon to surgeon.Angles at which grafts are placed, distribution of grafts; placement density will all decide the final results.

ANYTHING LESS THAN 3000 GRAFTS (5000-6000 HAIRS) WILL NOT GIVE GOOD DENSITY IN HAIR TRANSPLANT.

3000 grafts is an easy job in hands of a good surgeon. Surgeons with very low transaction rate (follicles cut at the time of extraction) can extract 3000-4000 grafts in less than 4 hours.

Inference is that if you are looking for hair transplant plan it with due research. In India there is trend for calling hair as grafts. Check for number of grafts (approx 2 hair per graft).

Never divide your procedure in two or three days. This can affect results as little swelling on scalp on next day of your procedure can change the angle of the new transplant later.Continous trauma and anesthesia is also not good for the already placed hair. Wait for at least 6 months before repeating the procedure

CHOI implanter is a small instrument used for hair transplant by few people around the world. I decided to try and see is it really a great instrument.

The device was a small pen with18 or 19 gauge needle fixed in the front. The graft has to be loaded in the front of this needle and the needle is used to create hole and implant the graft simultaneously. Used these instruments for few years and found that it is not a great instrument to be used in hair transplant. It’s much better to create recipient sites and implant compared to using implanter. I had following observation while using this instrument

1. It doesn’t change the procedure anyway in fact makes it more complicated. Ultimately a needle makes a hole and pushes the graft in the hole.

2. Its difficult to load grafts with multiple hairs without any friction or rubbing the follicles around the wall. Grafts without multiple hairs affect results.

3. A good perifollicular tissue is critical for the survival of the grafts. It’s difficult to load the graft in implanter with little extra tissue. It usually gets peeled off while loading.

4. Using the implanter was good for 1200 to 1500 grafts but it is almost impossible to do 3000-4000 grafts session with implanter.

5. Its difficult to create exact angles as the there is complete movement of hand to pick each and every graft loaded in implanter.

5. The needles remains ok for first few 100 grafts but as the needle gets blunt the force to push needle to create incision increases and there is huge popping of grafts. Changing the needle after every few hundred grafts is a costly affair.

6. Grafts cannot be transplanted very closely due to pressure created by the needle gets transferred to nearby follicles leading to pop up of existing follicles. This leads to the poor results and may need multiple sessions.

7. The implanter has to be disposable but cost doesn’t allow that. So usually needles are disposed and the plastic body is reused.

8. Single person has to work in scalp which increases the time required for the surgery.

9. The quality of implanter varies from manufacturer to manufacturer and those available in India are the worst.
